You’ll be asked about:

  • Your pregnancy history (how your previous pregnancies went, any C-sections, complications).
  • Medical records from your OB/GYN.
  • Lifestyle habits: Do you smoke, drink, or take any medications?

Health events: Things like surgeries, chronic conditions, or mental health history.

💡 Real talk: If you’re thinking “I don’t want to admit that I had postpartum depression” or “I used to take anxiety meds,” say it anyway. These things don’t automatically disqualify you. They just help doctors and coordinators make sure you have the right support.

🧠 The Mental Health Evaluation

This part surprises people but in a good way. It’s less like a test and more like a conversation. We will refer you to a psychologist who will chat with you (and often your partner, too) about:

  • Why you want to be a surrogate.
  • Who’s in your support circle.
  • Your personal family and partner relationship history.
  • How you handle stress, conflict, or tough moments.
  • What you expect from the pregnancy and after delivery.

📝 The PAI Test

Yes, there’s also a standardized test called the PAI (Personality Assessment Inventory). Don’t panic- it’s not an IQ test and there are no “right” answers.

It looks at things like:

  • Stress and anxiety levels
  • Mood and emotional balance
  • How you relate to others
  • How resilient you are under pressure

It’s basically a snapshot of your emotional well-being. And yes, some questions feel repetitive. That’s normal.
